FishBase historically catalogued this species under the genus Simpsonichthys as Simpsonichthys perpendicularis, reflecting earlier generic arrangements within the South American annual killifish clade. The reorganisation that placed it in Ophthalmolebias reflects phylogenetic work on the relationships among Brazilian annual killifish genera. The genus name Ophthalmolebias combines the Greek ophthalmos (eye) with lebias, a classical name applied to small cyprinodontiform fishes, drawing attention to the characteristically large eyes that are a feature of the genus.
