Phago boulengeri was described by the Dutch zoologist Anna Antonia Wilhelmina Schilthuis in 1891, part of a small wave of late-nineteenth-century descriptions of Congo basin fishes. It belongs to the family Distichodontidae — the African citharinoid characins that include the algae-grazing Distichodus and their fin-eating relatives — within the order Characiformes. The genus Phago is small and entirely African, restricted to a handful of species that share an elongated body and a specialised, forward-set mouth adapted for rasping fin and scale tissue rather than for grasping whole prey.

The Catalog of Fishes (Eschmeyer, California Academy of Sciences) is the governing authority for the valid name and combination; Phago boulengeri has not been subject to the kind of wholesale generic reshuffling seen elsewhere in Characiformes (for example the pacu and silver-dollar splits within Serrasalmidae), and it remains in its original genus. The species epithet honours the Belgian-British ichthyologist George Albert Boulenger, a major contributor to the description of African freshwater fishes in this period.
