Leporinus alternus was described by the American ichthyologist Carl H. Eigenmann in 1912, with a type locality at Tukeit in Guyana (holotype held at the Field Museum, FMNH 53361). It belongs to the family Anostomidae — the headstanders — within the order Characiformes, and to the large genus Leporinus, whose roughly 90 recognised species are defined by an elongate body, a small subterminal or slightly oblique mouth, and the head-down, oblique grazing posture that gives the group its common name.
The taxonomic status of L. alternus has not been entirely settled in the literature: some sources treat it as a synonym or close relative of L. maculatus, while others, including regional checklists of Guyana's freshwater ichthyofauna, continue to list it as a valid, recognized species. The genus name Leporinus is Latin for "hare-like," a reference to the incisor-like front teeth some early describers likened to a rabbit's.
