Leporinus trimaculatus was described by José Luiz de Figueiredo Garavello and Heraldo A. Britski Santos in 1992, and it sits in the genus Leporinus within the family Anostomidae, the headstanders, in the order Characiformes. Anostomidae takes its common name from the fish's characteristic feeding posture — an oblique, head-down angle used to graze algae and biofilm from submerged surfaces — a trait shared with the related genus Anostomus and, more loosely, with the smaller and gentler Chilodus (the spotted headstanders).
The genus Leporinus is large and taxonomically active; several members have been reshuffled among related anostomid genera as the family has been revised, and species-level identification within the genus is frequently confused in the aquarium trade because juveniles and adults can differ markedly in pattern. The specific epithet trimaculatus is Latin for "three-spotted," referring to the three dark blotches along the body that give the fish its common names of three-spot headstander or black-banded leporinus.
